General Summary
The Home Health RN acts as a primary care manager for a caseload of patients. The RN’s role is to assess, plan, implement, and evaluate the plan of care coordinating with the patient's physician. The case manager functions in an assigned territory within Ashtabula County.

Minimum Qualifications
Education, Knowledge, Skills and Abilities
- Degree in nursing required, BSN preferred.
- Strong clinical and assessment skills
- Ability to work independently
- Strong interpersonal communication skills
- Ability to document in an EMR
Required Length and Type of Experience
- One year nursing experience preferred
- New graduates welcome --Individualized Orientation Program with Mentor program for new graduates
Required Licensure or Certification
Current State of Ohio nursing license.
