Positive Engagement Specialist/Social Worker Resident Supports (Contract Position) Job Title: Positive Engagement Specialist, Resident Supports (Contract Position) Location: Stonecrest Living, Minnetonka, MN Employment Type: Full-Time/Full-Time Contract Reports To: Director of Resident Services / Behavioral Health Manage Job Summary Stonecrest Living is seeking a compassionate and strategic Positive Engagement Specialist to join our team in Minnetonka. This contract position focuses on enhancing the lived experience of residents with Serious and Persistent Mental Illness (SPMI) and disabilities in our 245D licensed and assisted living programs. The specialist will lead efforts to reduce challenging behaviors by developing and supporting PEPs (PEPs)—individualized behavior support strategies rooted in trauma-informed and person-centered care. Most of the time will be spent interviewing residents and staff to understand needs, preferences, and barriers to engagement so PEPs can be developed. Overtime, these insights will also inform coaching and staff training to ensure that each resident receives respectful, empowering support tailored to their goals. The role also involves writing and sharing the final PEPs (PEP) with staff, families, and interdisciplinary care teams as appropriate, using digital tools and documentation platforms to ensure clarity, accessibility, and compliance. Key Responsibilities • Plan Development, Documentation & Communication (50%) o Write individualized PEPs based on resident interviews, staff input, and observed behaviors o Ensure plans meet DHS guidelines for person-centered thinking and planning requirements o Share finalized plans with direct care staff, families, and interdisciplinary care teams using digital platforms and with adherence to HIPPA policies and procedures o Use tools such as Microsoft Word, Canva, and Adobe to create clear, engaging, and accessible documents to support successful implementation of PEPs -Maintain organized records and contribute to EHR systems when applicable • Resident & Staff Interviews (30%) -Conduct structured and informal interviews with residents to understand behavioral triggers, unmet needs, and personal goals so plans reflect their dreams, goals, needs and experiences. -Engage staff in reflective conversations to identify challenges and opportunities for development of PEPs -Translate findings into actionable strategies that inform PEPs and team practices -Coaching, Training & Observation (20%) -Observe staff-resident interactions to assess implementation of PEPs if questions or issues arise. Provide real-time coaching and feedback to staff on trauma-informed, person centered approaches Collaborate with supervisors to reinforce skill development and ensure consistency across shifts Participate in interdisciplinary team meetings to share insights and advocate for resident-centered approaches Support training initiatives that promote empathy, de-escalation, and proactive engagement. Qualifications • Licensed social worker (LPCC, LICSW, LMFT, LP) REQUIRED. • Minimum of 2–3 years’ experience working with youth and adults with SPMI and disabilities in 245D licensed programs and assisted living facilities • Strong understanding of trauma-informed care, behavioral health, and person-centered planning • Excellent communication, observation, and coaching skills • Ability to build trust with diverse populations and navigate complex interpersonal dynamics • Familiarity with documentation standards and regulatory compliance in residential settings • Experience in behavioral support or mental health (e.g., CPI, ARMHS, DBT) • Experience with motivational interviewing or restorative practices • Bilingual or multilingual abilities are a plus, but not necessary o Experience with Electronic Health Record (EHR) systems preferred but not required o Strong working knowledge of Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) o Ability to use Canva for visual communication and training materials o Familiarity with Adobe tools (such as Acrobat, Photoshop, or Illustrator) for documentation and creative support o Comfortable navigating cloud-based platforms and collaborative tools (e.g., SharePoint, Microsoft One Drive, Teams, Google Workspace) About Stonecrest Living Stonecrest Living has built a strong reputation as a leading provider of specialized care for children, youth and adults with complex neurodivergent needs and behavioral challenges. Current services include crisis respite at home, Individualized Home Supports (IHS) with training, night supervision, Community Residential Services (CRS) homes and an assisted living facility in Minnetonka. Our leadership team brings a collective experience of over 50 years in both residential and community-based services, with a proven history of working with challenging resident populations. Supporting residents with behavioral and other challenges requires staff who have expertise in human development as well as interventions that address neurodivergent needs and complex behaviors. Stonecrest Living specializes in providing intensive residential services for individuals with complex medical needs and behavioral health conditions.
