Careforce Homehealth, Inc. - Home Health Occupational Therapist Our growing healthcare organization is seeking a dedicated and compassionate Home Health Occupational Therapist to serve patients in Cook, Will, DuPage, Lake, McHenry, and Kane counties. Job Summary The Occupational Therapist will assist patients in regaining or enhancing their physical and mental abilities while adapting to disabilities. Responsibilities include implementing therapy programs that incorporate functional skills, daily living activities, and rehabilitative techniques to promote independence. Qualifications - Graduate of an accredited occupational therapy program with a valid OT license in Illinois. - Minimum of one year of experience in home health or rehabilitation settings (preferred). - Strong initiative and independent judgment skills. - Skilled in ADL training, adaptive equipment recommendations, and patient education. - Excellent communication and interpersonal skills. Key Responsibilities - Deliver therapy services following the prescribed plan of care. - Assess and regularly evaluate patient needs and progress. - Provide specialized occupational therapy techniques to improve patient mobility and functionality. - Implement preventive and rehabilitative procedures as required. - Maintain detailed clinical notes and progress documentation for each visit. - Coordinate interdisciplinary care with physical therapists, speech therapists, and medical social workers. - Keep healthcare teams updated on changes in patient conditions and treatment needs. - Participate in care planning, peer reviews, and performance improvement initiatives. - Comply with all agency policies and procedures. Work Environment - Travel required to provide in-home patient care. Supervision & Reporting - Reports to: Director of Patient Care Services/Agency Supervisor. Physical Requirements - Ability to stand, lift, bend, and perform physical activities for extended periods. - Must be able to perform heavy lifting and support patient mobility as needed. Job Type & Compensation -Per-Visit Pay: Starting at $65.00 per hour. - Medical Specialty: Home Health. - Work Setting: Patient’s Home.
