This family owned home Health Agency (est. 1996) is launching and expanding a Remote Patient Monitoring (RPM) program serving patients with chronic conditions such as hypertension, diabetes, and COPD.
We are seeking a dependable, highly organized professional to lead and grow this program.
This is a full-time, in-office role in San Antonio.
The primary focus of this position is to build, manage, and scale our RPM program.
During the initial ramp-up phase, you will also support select home health office operations.
As RPM census grows, this role will transition into a fully dedicated RPM leadership position.
Position Overview
You will be responsible for the day-to-day operations of our Remote Patient Monitoring service line ensuring compliance, patient engagement, physician coordination, and program growth.
This role requires strong organization, follow-through, and confidence communicating with both patients and physicians.
Primary Responsibilities Remote Patient Monitoring
• Enroll patients and physicians into the RPM program
• Educate patients and families on device usage
• Coordinate device delivery and installation
• Monitor daily vitals within RPM platform
• Document outreach and escalate abnormal readings per protocol
• Manage Medicaid and MCO prior authorizations and renewals
• Perform monthly eligibility checks
• Track device inventory (deliveries, returns, replacements)
• Generate audit-ready reports for compliance
• Serve as primary point of contact for participating physicians
• Help scale program toward 300400+ active patients
Secondary Responsibilities (Initial Ramp-Up Phase)
• Assist with intake and referral coordination
• Review consents and physician orders
• Support documentation review and audit readiness
• Follow up with physician offices for signed orders
• Assist DON and administrative team as needed
As RPM census increases, this role will transition to focus primarily on telehealth operations.
Qualifications
• Active Texas LVN license preferred (strong healthcare operations candidates considered)
• 2+ years experience in home health, primary care, care coordination, or telehealth
• Experience with Medicaid/MCO authorizations strongly preferred
• Strong EMR experience required (Axxess, WellSky, KanTime, or similar)
• Highly organized and detail-oriented
• Comfortable managing multiple tasks and deadlines
• Excellent communication skills with physicians and patients
• Bilingual English/Spanish strongly preferred
• Must be based in San Antonio
Compensation
$30$33 per hour depending on experience
