About the position
1. Driving Behavior Change
Provides coaching to reduce and/or eliminate high-risk behaviors to
accomplish improved health in a large group or individual setting. Applies
evidence-based strategies, behavior change, motivational interviewing
skills, and guidance for adoption of specific behaviors shown effective by
research.
Coaches members towards solution-based interventions with touchpoints to
meet goals independently.
Communicates with members digitally to enhance member impact on their
goals and increase engagement.
Effectively engages members on a large scale while managing a revolving
caseload and educating and guiding members towards meeting their wellbeing
goals.
Performs member coaching at health fairs and community events while
following HIPAA requirements.
Partners with and empowers the member to develop an actionable care plan
that helps prevent avoidable utilization.
2. Quality Improvement & Problem Solving
Engages in quality improvement initiatives. Supports leadership in
system-level improvements. Analyzes dashboard data for program
enhancement. Mentors new staff to promote best practices.
Identifies new problems and creates solutions in a changing and evolving
internal and external healthcare operating environment. This position
requires moderate level of autonomy in decision making.
3. Performs all other miscellaneous responsibilities and duties as assigned or
directed.

Requirements
• Bachelor's degree and two years of related work experience; or equivalent combination of education and work experience
• Effective communication skills.
• Must meet facility specified health screening/annual competency requirements.
• Intermediate working knowledge in Microsoft Office applications, including but not limited to Word, Excel, Outlook, and PowerPoint.
• Experience working with electronic medical records (i.e., EPIC, HHIE).
• Reliable transportation to perform in person (Face to Face) meetings with members in their home or in alternative settings.
