Experienced Medical Scribe Needed | SOAP Notes ICD, CPT | Outpatient Primary Care | NextGen - Contract to Hire

I’m an Adult Primary Care Provider running a growing private medical office in Brooklyn, NY. We treat adult patients with both chronic and acute conditions, perform in-office procedures (e.g., joint injections, migraine management, ultrasound, vaccinations), and document in NextGen EHR. I am seeking a responsible, detail-oriented medical scribe to produce high-quality, individualized SOAP notes. ⚠️ This is not a copy-paste or template-filling role. Every record must be carefully written, medically accurate, with correct ICD-10 and CPT codes and insurance-ready. What You’ll Do Write unique, accurate SOAP notes for each encounter. Document CC, HPI, ROS, PE, Assessment, Plan with clear medical logic. Assign and document proper ICD-10 diagnoses and CPT procedure codes for every visit. Ensure correct visit time documentation that matches the billed CPT level. Capture medical necessity for every procedure performed. Maintain strict confidentiality and HIPAA compliance. Learn quickly and apply corrections after the first feedback. Repeated mistakes will not be tolerated. If you do not pay attention to ICD-10, CPT, and medical necessity — this role is not for you. Quality Benchmarks Initial visits: expanded ROS (≥12 systems) and comprehensive PE. Procedures: always tied to a clear ICD-10 medical reason. Accuracy: notes with wrong visit time, missing diagnoses, or incomplete documentation = unacceptable. Must write in clean English, free of grammatical errors, with professional clinical terminology. Must-Have Strong written English. Solid knowledge of medical terminology, ICD-10, and CPT coding. Prior experience with SOAP notes and/or medical scribing. Familiarity with NextGen EHR is a big plus. Strong computer literacy and ability to work independently. Schedule & Payment Starting workload: ~5–10 hours/week, with the possibility to expand if collaboration goes well. Flexible hours; long-term collaboration. Hourly rate: $10–12 USD (based on skills and experience). Starting rate is $10/hr. Strong candidates who consistently produce high-quality, error-free notes may be moved up to $12–13/hr with performance bonuses. Performance bonuses available for accuracy, consistency, and quality.
