Behavioral Health Outreach Care Specialist

About the position The Behavioral Health Outreach Care Specialist at Elevance Health is responsible for managing treatment plans for members with less complex cases, ensuring compliance with medical guidelines, and proactively identifying opportunities to enhance member health and meet quality goals. This hybrid role involves both remote work and field visits, requiring candidates to reside in Ohio and be available for community outreach. Responsibilities • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs. • Assesses member compliance with medical treatment plans via telephone or through on-site visits. • Identifies barriers to plan compliance and coordinates resolutions. • Identifies opportunities that impact quality goals and recommends process improvements. • Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider. • Coordinates identification of and referral to local, state or federally funded programs. • Coaches members on ways to reduce health risks. • Prepares reports to document case and compliance updates. • Establishes and maintains relationships with agencies identified in appropriate contract. Requirements • High school diploma or GED • Minimum of 1 year related experience • Bilingual preferred • Experience in behavioral health strongly preferred • Experience with mental health and substance use disorders preferred • Case management experience • Community Health Workers, Certified Nurse Assistant, or Certified Medical Assistant experience preferred • Bachelor's degree in a related field preferred • Strong oral, written, and interpersonal communication skills • Problem-solving skills • Facilitation skills • Analytical skills Nice-to-haves • Behavioral health experience • Experience with mental health and substance use disorders • Case management experience • Community Health Workers experience • Certified Nurse Assistant or Certified Medical Assistant experience • Bachelor's degree in a related field Benefits • Employee stock purchase plan • Paid holidays • Disability insurance • Health insurance • Dental insurance • Paid time off • Vision insurance • 401(k) matching • Life insurance Apply tot his job
